Health District of Northern Larimer County

The Health District of Northern Larimer County provides a variety of services to help improve the health of the community. The district encompasses the northern two-thirds of Larimer County, including Fort Collins, Laporte, Wellington, and Red Feather Lakes. Services include the Family Dental Clinic; Connections to assist people in finding mental health care; a quit-smoking program; other preventive services including cholesterol testing and counseling; prescription assistance; the Larimer Advance Care Planning Team to help people prepare their advance directives (living will, medical durable power of attorney); and Larimer Health Connect to help people find health insurance or apply for Health First Colorado (Medicaid) or CHP+. Many services are on a sliding-fee scale so that low-income residents can get needed health care.

Health District Prescription Assistance

The Health District's Prescription Assistance Program works with low-income people who need help paying for their prescription medicines. The program is open to residents of the Health District, including Fort Collins and northern Larimer County. To be eligible for the program, you must have a prescription from your doctor and meet income guidelines. We can help by issuing vouchers for medications, and by working with drug manufacturers to get free or discounted medications, or co-pay cards.

Please call (970) 416-6519 for more information and to find out if you may be eligible for the program. The Health District does not charge a fee to help you enroll in these programs; but in some cases you may have a small charge at the pharmacy.

Connections - Health District of Northern Larimer County and SummitStone Health Partners

Connections is a partnership of the Health District and SummitStone Health Partners.

Connections can help with:
- Identifying counseling and treatment options
- Referrals to local counselors, treatment programs, classes and support groups
- Crisis intervention
- Information on mental health and substance abuse
- Coordinating care
- Locating services that meet the client's needs
- Finding affordable counseling and medications
- Psychological testing for children and adolescents

Health District Cholesterol/Blood Pressure Testing

The Health District offers cholesterol and blood pressure testing at many locations in the community. Results for total cholesterol, HDL, LDL, triglycerides, and blood glucose are available in a 25-minute appointment. A registered nurse will explain your results and offer health information. People who have never had a cholesterol test from the Health District are eligible to receive their first test for free. Cost for all other clients is $15, and reduced fees are available to residents of northern Larimer County depending on income. Health District Dental Services

The Health District Family Dental Clinic is a friendly, modern, and comprehensive dental clinic in north Fort Collins. We provide free and low-cost dental services to adults and children who have trouble affording care. If you have been putting off going to the dentist because of cost, we may have an option for you.

The clinic serves low-income families who live within the Health District (northern two-thirds of Larimer County) and who have no dental insurance or who are covered by CHP+ or Health First Colorado (Medicaid). Our dental services include preventive care and cleanings, dentures, fillings, and some other procedures; we do not provide orthodontic care (braces).

Health District Quit Tobacco Program

Ready to quit smoking? We can help! The Health District offers group classes and individual support, as well as free nicotine patches, gum or lozenges while you're in the program. Individual sessions are available at any time and six-week classes are available periodically throughout the year. Our experienced Tobacco Treatment Specialists have a positive, supportive approach with proven techniques for quitting smoking/quitting tobacco use. Sliding-fee scales are available for Health District residents. Call for a free session to see if the program is right for you. Health District Larimer Health Connect

Larimer Health Connect provides free, in-person assistance to help individuals, families, and small businesses understand and choose health coverage. We will also be able to determine your eligibility for Health First Colorado (Medicaid) and CHP+ and can help you apply for those programs if you qualify. Our Health Coverage Guides are certified through Connect for Health Colorado, the statewide online health insurance marketplace. Guides can help you shop for coverage and complete the application process for health insurance offered through the marketplace, as well as go over the financial assistance options that are available only through Connect for Health Colorado. Help is available throughout the year. Larimer Health Connect staff can help you choose health insurance during the open enrollment period in the late fall, and we can help with insurance options if you've recently lost your job or gotten a new job, or if you've experienced a change such as getting married or divorced, having a child, or moving to the area. If you are eligible for Medicaid or Child Health Plan Plus (CHP+), you can apply for those at any time, and Larimer Health Connect staff can help with the application process.
