HealthInfoSource.com was created with this in mind—to assist you in finding providers and resources that will help you on your journey.
On this site, you’ll find information about local behavioral health providers, organizations, services, support groups, and classes. You’ll also find health guides and articles that will answer some of your questions and help you make decisions about mental health and/or substance use care.

About the Health District
The Health District of Northern Larimer County has been meeting local healthcare needs since 1960. Today we have programs that help local residents in accessing affordable dental care, behavioral health care, and help finding health insurance and affordable prescriptions. We also join with other organizations to assess and plan for the larger healthcare needs of our community.
The Health District is a special tax district—like a school, fire, or water district—that was created by voters to serve the health needs of our community. The boundaries of the district encompass the northern two-thirds of Larimer County and include the cities of Fort Collins, Laporte, Timnath, Wellington, Livermore, and Red Feather Lakes.
The Health District is supported by local property tax dollars, as well as through grants, partnerships, and other funding. It is governed by a publicly elected, five-member board of directors that establishes the agency’s priorities.
For mental health and substance use care, our Connections Program helps individuals, couples, and families find services to meet their needs, and provides education and support throughout the process. Please note that Connections is not a crisis service. In a behavioral health emergency, call the SummitStone Crisis Line 24/7 at 970-494-4200, ext. 4 or call 911.
Connections Adult Services
We provide answers, options, and support to help adults find treatment for mental health and substance use concerns. We can answer your questions, offer initial screening to identify your specific needs, and provide referrals to therapists in the community. Child, Adolescent, and Young Adult Connections (CAYAC)
We support families in navigating the network of behavioral health services available to youth in Larimer County, while bridging gaps in care. CAYAC helps young people (up to age 18) and their families with the early identification, assessment/testing, and treatment of mental health and substance use concerns.
