Health District Mental Health and Substance Use Services

Connections Adult Services

Connections offers answers, options, and support to adults looking for help with mental health or substance use concerns, such as depression, anxiety, and drug or alcohol use. Services include needs assessment, information and referral, assistance with coordinating and navigating care, brief intervention, connection to reduced-cost counseling, education, and training. Child, Adolescent, and Young Adult Connections (CAYAC)

The CAYAC team supports families in navigating the network of behavioral health services available in Larimer County, while bridging gaps in care. CAYAC helps young people (up to age 18) and their families with the early identification, assessment/testing, and treatment of mental health and substance use concerns.

The Health District of Northern Larimer County has been meeting local healthcare needs since 1960. Today we provide services for dental, mental health, prescription assistance, cholesterol and blood pressure testing, and help finding health insurance for the residents of northern Larimer County. We also join with other local organizations to assess and plan for the larger healthcare needs of our community.

In addition to the behavioral health services available at Connections, the Health District offers the following services:

Family Dental Clinic
The Health District Family Dental Clinic in Fort Collins provides quality, free and low-cost dental care to people in northern Larimer County who do not have dental insurance. We take Medicaid and CHP+. And we offer low sliding fees to those who qualify. Affordable dental insurance for seniors may also be available. Larimer Health Connect
Our Certified Health Coverage Guides offer free in-person or virtual help to answer your questions about health insurance; review coverage options so you can make an informed choice that's best for your situation; complete the application process for health insurance offered through the Marketplace; learn about financial assistance options available through Connect for Health Colorado; determine your eligibility for Health First Colorado (Medicaid) and CHP+; compare Medicare plans and apply; and complete the application process for health insurance through Colorado Connect, including OmniSalud plans.

Prescription Assistance
The Prescription Assistance program helps people with limited incomes fill their prescriptions through vouchers and applications to drug company programs. Call the Prescription Assistance office at 970-416-6519 to schedule an eligibility screening. You will need to provide information about your income, address, and medications.

Heart Health Promotion
This program offers free and low-cost cholesterol, blood glucose, and blood pressure testing several times each month in the Fort Collins area, focusing on people who have not had their cholesterol tested in the past year. We also hold clinics for groups who may lack access to these important tests, including Spanish-speaking communities, residents of permanent supportive housing, and people experiencing homelessness. Salud provides medical, dental, pharmacy, and behavioral health services for all ages to keep families and individuals healthy. The clinic accepts Medicaid, Medicare, CHP+, and most private insurance plans. Salud offers a sliding-fee scale based on family size and income. Insurance enrollment assistance is available.

The Loveland Community Clinic is a community health center, open Monday–Friday, providing affordable, quality care. Christ Clinic provides free primary medical care in an outpatient setting. Its aim is to act as the essential medical provider for members of the northern Colorado community who are uninsured or do not have access to other providers, regardless of their background, creed, or beliefs. This includes young adults, families with children, and adults with chronic diseases who are not yet eligible for Medicare. We do not provide urgent care or emergency services.

St. Matthew’s Medical Clinic is a nonprofit, faith-based organization established to provide free medical services to children and adults in the community. We offer preventive care and treatment/management for acute and chronic diseases. Behavioral health services are also available on a limited basis. All patients are welcome at St. Matthew’s regardless of their age, race, gender, religious preference, or financial status. We do not provide urgent care or emergency services.

The organization works to strengthen and empower immigrant communities and their families through key services, civic education, leadership development, and community organization. Alianza NORCO offer drivers license information, legal information, and other services.

 

United Way 2-1-1
211 is for non-emergency assistance only. Callers needing emergency medical, fire, or police aid should always call 911.

United Way’s 211 call center is staffed by information and referral specialists who assist the public in finding community services. People contact 211 for information about childcare referrals, disaster recovery, donation referrals, food and housing assistance, and other important services.
Hearing-impaired callers can reach 211 by calling Relay Colorado (711) or 1-800-659-2656.
