Ascent at SummitStone Health Partners is a unique program serving 15-29 year olds in Larimer County who are experiencing their first psychotic episode or symptoms (symptoms have started within the past two years).
This specialized program provides coordinated services designed to increase independence in a recovery model. Clients receive treatment for up to 3 years from an interdisciplinary team who provide therapy, support for employment and educational goals, assistance in managing symptoms, as well as help with medication and primary care coordination. The program offers wrap-around services from a multidisciplinary team that includes a therapist, case manager, peer support specialist, a psychiatrist, and a nurse. The goal of this program is to improve outcomes for youth and young adults, as well as support for their families/friends. We take all insurances and have funds available to assist those without insurance.
